  2. From: the originator of the form contents.
  3. County: the county that the location of the damage resides.
  4. Date: current date.
  5. Time: current time.
  6. Location Coordinates: the latitude and longitude of the delivery site. Please use 4 to 5 decimal digits right of decimal pt.
  7. What Happened: a concise description of what damage has happened. Required field.
  8. When: date and time of when the damage occurred. Required field.
  9. Where: street address and/or nearest intersection or mile marker. Required field.
  10. Who affected: number of persons injured or affected by the damage. Required field.
  11. Any injuries: type and extent of any known injuries. Required field.
  12. Recorded by: optional field of radio operator recording this report and their call sign.
