Damage Report Form Instructions
- Incident name: name of the incident or event (it will be saved for the next time the form is used).
- From: the originator of the form contents.
- County: the county that the location of the damage resides.
- Date: current date.
- Time: current time.
- Location Coordinates: the latitude and longitude of the delivery site. Please use 4 to 5 decimal digits right of decimal pt.
- What Happened: a concise description of what damage has happened. Required field.
- When: date and time of when the damage occurred. Required field.
- Where: street address and/or nearest intersection or mile marker. Required field.
- Who affected: number of persons injured or affected by the damage. Required field.
- Any injuries: type and extent of any known injuries. Required field.
- Recorded by: optional field of radio operator recording this report and their call sign.
